Once you decide that tubal reversal surgery is an option that you want to consider, we will want to get as much information from you ahead of time as possible.
Please go to the forms section of our website and fill out as much as you can and submit it. You must provide your current height and weight in order for our surgeons to determine if they can perform a tubal reversal procedure on you in our surgery center. Falsifying information on these forms may result in the cancellation of your surgery and forfeiture of fees. Please note, you will be weighed on the day of surgery.
We will need a copy of your operative and/or pathology reports from your tubal ligation surgery. By studying those reports, we will be able to tell you whether a tubal ligation reversal is likely to be successful and safe. If you are not able to get copies of your operative or pathology reports, we can still perform a simple and safe diagnostic procedure to determine the extent of any damage to your fallopian tubes caused by the tubal ligation. Our surgeons also perform Essure and Adiana reversal surgery.
In addition, if our surgeons find uterine fibroids and/or uterine polyps, they will remove them because they can interfere with pregnancy. Not removing these non-cancerous growths can reduce your chances of a successful tubal reversal surgery and future chances of a successful pregnancy. Because either of these cases are gynecologic issues, most health insurance plans will cover the cost of the removal unless the patient has not met her deductible. Patients with no insurance would be expected to pay for the procedure out of pocket.
We also encourage you to have your partner do a semen analysis before you undergo tubal reversal surgery.
